If your toolbar is not loading after a re-boot then try the suggestion below.

Outlook 2003
If not then open Outlook and go to the Help-About MS Outlook menu and select disabled items.  If REMSCore.dll is listed there then enable it and restart outlook.
 
Outlook 2007
Make sure it is not disabled.  In Office 2007 look under tools-trust center-COM Addins, then look for disabled items.  If it is listed in the disabled items then select disabled items from the manage section at the bottom and enable it.
 
Outlook 2010
Make sure it is not disabled.  In Office 2010 look under File-Options-trust center-COM Addins, then look for disabled items.  If it is listed in the disabled items then select disabled items from the manage section at the bottom and enable it.
