If your toolbar is not loading after a re-boot then try the suggestion below.

Outlook 2002-2003

From  within MS Outlook go to the Help-About MS Outlook menu and select disabled items.  If REMSCore.dll is listed there then enable it and restart outlook.

Outlook 2007

In Office 2007 look under tools-trust center-COM Addins, then look for disabled items.  If it is listed in the disabled items then select disabled items from the manage section at the bottom and enable it.
